    Background

    Camp Eden will run background checks. Criminal charges or convictions are not an automatic bar to employment. The camp will consider the nature of the offense, the date of the offense, and the relationship between the offense and the position being applied for.

    Application/Employment Statement

    It is understood that this application and records become the property of Camp Eden, which reserves the right to accept or reject it. I further agree to abide by all rules, regulations, and policies of the camp. I agree to any medical examination or drug testing which may be required upon offer of employment and understand that my employment is contingent upon successful completion of the examination. I understand that employment is “at will” and may be terminated by Camp Eden or myself at anytime.

    I have applied to Camp Eden for employment and authorize them to contact references, past or present employers, and any other source of information which may be relevant to my application. I authorize these references, employers, and sources to furnish any and all information requested. Furthermore, I hold harmless and release the same from all liability for any damage associated with providing this information. This authorization voids any prior or written agreements limiting release of said information.

    I understand Camp Eden is a conservative Christian camp serving churches from the West. I am convinced my reputation, current lifestyle, personal standards and theological beliefs would be consistent with Camp Eden and its constituency.

    Perjury Statement

    “Any applicant who knowingly or willingly makes a false statement if material fact or thing in the application is guilty of perjury in the second degree as defined in Section 18-8-503, C.R.S., and, upon conviction thereof, shall be punished accordingly.”

    Child Abuse Reporting Statement

    Under the “Child Protection Act of 1987” (C.R.S. 19-3-301) in the Colorado Children’s Code, child care center workers are required to report suspected child abuse or neglect. The law at 19-3-304 states that if a child care worker has “reasonable cause to know or suspect that a child has been subjected to circumstances of conditions which would reasonably result in abuse or neglect shall immediately report or cause a report to be made of such fact to the county department or local law enforcement agency.”

    “Abuse” or “child abuse or neglect” means an act of omission in one of the following categories which threatens the health or welfare of a child: skin bruising, bleeding, tissue swelling, or death; any case in which a child is subjected to sexual assault or molestation, sexual exploitation, or prostitution; any case in which a child is in need of services because the child’s parents, legal guardian, or custodian fails to take the same actions to supervision that a prudent parent would take.

    If at any time a staff member reasonably suspects child abuse, it is the responsibility of that staff member to report or to cause a report to be made of this suspicion to the local county department of social or human services at Camp Eden or the police department. It is not the staff’s role to investigate suspected abuse—only to report it. Persons who make a good faith report are immune from civil and criminal liability. Additionally, the law provides for the protection of the identity of the reporting party.

    A child care worker who fails to report suspected child abuse or neglect commits a class 3 misdemeanor and will be punished as provided in section 18-1-106, C.R.S. The staff person could also be liable for damages “proximately caused thereby.”

    Statement of Affirmation

    I hereby affirm that I have not been convicted or entered into a differed judgment for any of the following crimes: incest, child abuse, child sexual behavior, murder; any crime involved with child prostitution or the sale or possession of sexually explicit materials harmful to children; any crime which adversely reflects upon my character and suitability as a resident of a family day care home; or any crime which might indicate that I pose a threat to the health, welfare, and safety of children.

    Background Check Release

    As part of the employment process, Beth Eden Ministries, will obtain a consumer report, which I understand may include information regarding my credit worthiness, credit standing, credit capacity, character, general reputation, personal characteristics, or mode of living.

    Authorization
    During the application process and at any time during the tenure of my employment with Beth Eden Ministries, I hereby authorize ChoicePoint WorkPlace Solutions Inc., on behalf Beth Eden Ministries to procure a consumer report which I understand may include information regarding my credit worthiness, credit standing, credit capacity, character, general reputation, personal characteristics, or mode of living. This report may be compiled with information from credit bureaus, court record repositories, departments of motor vehicles, pastor or present employers and educational institutions, governmental occupational licensing or registration entities, business or personal references, and any other source required to verify information that I have voluntarily supplied. I understand that I may request a complete and accurate disclosure of the nature and scope of the background verification, to the extent that such investigation includes information bearing on my character, general reputation personal characteristics or mode of living.
